Pyoderma gangrenosum is an ulcerative process of unknown cause associated with systemic disease (e.g., inflammatory bowel disease [IFB] and arthritides) in roughly 50% of patients. Approximately one third of patients exhibit lesions at the site of injury. Postoperative PG is often a difficult diagnosis to make, particularly for nondermatologists. Misdiagnosis is frequent, prompting antibiotic treatment and repeated debridement before dermatologic referral. Postoperative PG can occur at any site but is most frequent on the breasts. Like classical PG, it may be associated with underlying disease processes.
