Recent studies have highlighted the high recurrence rate after surgical correction in secondary or functional mitral regurgitation (MR; NEJM JW Cardiol Jan 2016 and N Engl J Med 2016; 374:344), but less is known about the recurrence rate after repair of primary (degenerative) MR. In this single-center, multisurgeon report, investigators analyzed the long-term outcomes of 1218 patients undergoing surgical repair between 1990 and 2000 (mean age, 64; 29% women).
Left ventricular (LV) function was preserved (mean ejection fraction, 63%), with a mean LV end-systolic dimension of 37 mm. Prolapse was localized to only the posterior leaflet in 62% of patients, to only the anterior leaflet in 12%, or to both in 26%.
