Incidental pancreatic cysts are identified frequently when patients undergo abdominal imaging, but malignant transformation is uncommon (NEJM JW Gen Med Nov 15 2013 and Am J Gastroenterol 2013; 108:1546). Guidelines vary on recommendations for managing small asymptomatic pancreatic cysts without high-risk features. For example, the American Gastroenterological Association recommends follow-up magnetic resonance imaging (MRI) at 1, 3, and 5 years (NEJM JW Gastroenterol Jun 2015 and Gastroenterology 2015; 148:819), whereas the American College of Radiology recommends only a single follow-up MRI at 1 year, after which surveillance can cease if the cyst hasn't changed (J Am Coll Radiol 2010; 7:754).
