A randomized trial failed to show superiority or noninferiority for aliskiren, alone and in combination with enalapril, compared with enalapril alone.
Aliskiren (Tekturna), a direct renin inhibitor, is approved as an antihypertensive agent, but there are concerns about its use with angiotensin-converting–enzyme inhibitors or angiotensin-receptor blockers, particularly in patients with renal impairment. In the Aliskiren Trial to Minimize Outcomes in Patients with Heart Failure (ATMOSPHERE), an industry-sponsored study, 7000 patients who had chronic heart failure (New York Heart Association class II to IV; ejection fraction ≤35%) were randomized, after a run-in phase, to treatment with aliskiren, enalapril, or a combination of both drugs.
