In a cohort study, BPs lower than 120 mm Hg were associated with fewer adverse CV outcomes but higher 5-year all-cause mortality.
Guidelines recommend a systolic blood pressure (BP) treatment goal of <140 mm Hg in patients with type 2 diabetes (NEJM JW Gen Med Jan 15 2014 and JAMA 2014; 311:507), because randomized-trial results to support a lower BP goal are lacking, and evidence suggests a J-shaped relation between BP and adverse cardiovascular (CV) events. In this study, Swedish researchers used population-based registry data (on 187,000 patients [age, ≤75] with type 2 diabetes and no previous CV disease) to compare risks in patients whose systolic BPs met guideline recommendations (i.e., <140 mm Hg) with risks in patients whose BPs were higher or lower.
