Even with frequent blood-glucose monitoring and insulin injections, patients with type 1 diabetes often do not achieve euglycemia and frequently experience hypoglycemia. Glucose control is even more challenging during pregnancy. In sensor-augmented insulin pump therapy (SAIPT), sensors continuously monitor interstitial glucose to help regulate the administration of insulin with a pump; still, the patient must use the glucose measurement to adjust the insulin dose (which is particularly inconvenient at night). By incorporating a computer between the sensor and the pump (closed-loop insulin delivery [CLID]), glycemic control can be improved. However, most studies of CLID have been performed in nonpregnant patients over short periods.
