An HIV-infected patient with suppressed HIV-1 viral load on ART received a liver transplant in 2011 from an HIV/HCV-coinfected donor and developed transient donor-derived HIV viremia that was promptly suppressed with ART.
Experience with liver and kidney transplantation into HIV-infected recipients has been promising, with outcomes approaching those in HIV-negative recipients. In 2015, President Obama signed the HOPE (HIV Organ Policy Equity) Act, which allows transplantation in the U.S. using organs from HIV-infected donors into HIV-infected recipients. However, little experience with HIV–positive-to-positive liver transplantation has been reported in the scientific literature. This case report from the U.K. details the course of a patient with HIV and hepatitis C virus (HCV) coinfection who also had hepatocellular carcinoma.
