Still-Unproven Benefit of Systemic Antifungal Treatment for Candida Peritonitis

Survival rates ranged narrowly (from 67% to 72%) whether antifungal treatment was empiric or targeted — and whether Candida peritonitis could be confirmed.

Patients with candidemia benefit from early initiation of systemic antifungal treatment (NEJM JW Infect Dis Jun 2014 and Crit Care Med 2014; 42:1423). The possible benefit of early or even empiric antifungal treatment for Candida peritonitis (CP) has been suggested. To assess that possibility, researchers conducted a subgroup analysis of 279 adult enrollees in a prospective, multicenter observational study in France. All had received systemic antifungal therapy (SAT), after undergoing abdominal surgery for suspected or proven CP, during or <48 hours before their intensive care unit (ICU) stay.
