Despite the introduction of next-generation androgen receptor (AR)–targeted agents for patients with metastatic prostate cancer, resistance — either primary or acquired — remains a major therapeutic challenge. Up to 15% of patients treated initially with either abiraterone or enzalutamide have de novo resistance, and for patients progressing on one of these agents, the response rate to the alternative agent is in the 15% to 30% range.
