Proactive Antibiotic Stewardship

An active intervention reduced both the time to appropriate antibiotic therapy and mortality caused by bacteremia.

Timely administration of appropriate antibiotic treatment has a substantial impact on mortality in patients with septic shock (NEJM JW Infect Dis Oct 2014 and Crit Care Med 2014; 42:1749). To decrease the time to first dose of appropriate antibiotic therapy (FDAAT) for patients found to have bacteremia, the antibiotic stewardship program of a community teaching hospital implemented a novel protocol: An infectious disease pharmacist received pager notification of positive Gram stain results of blood cultures, reviewed the patient's current antibiotic therapy with the responsible physicians, and if necessary delivered the initial antibiotic dose to the patient's nurse to administer.
