Care transformation to improve outcomes, safety, and population health is a national imperative and is increasingly linked to reimbursement. To improve childhood asthma care, the American Academy of Pediatrics (AAP) organized a national learning collaborative based on the National Heart Lung and Blood Institute's 2007 asthma guidelines. Several AAP chapters in nine states were enrolled in four waves over 8 years, and they, in turn, enrolled individual pediatric practices. The national group developed standard encounter forms, patient registries, workflow assessments, and tools for patient self-management to be implemented by the practices.
