New Guideline for Therapeutic Drug Monitoring in Inflammatory Bowel Disease

Background and Objective

Therapeutic drug monitoring (TDM) in inflammatory bowel disease (IBD) is increasingly endorsed by experts. This new guideline on TDM covers both anti–tumor necrosis factor (TNF) agents and thiopurines. Data were considered insufficient to make recommendations on vedolizumab and ustekinumab.

Key Recommendations

• Consider TDM in adult patients with active IBD who are on anti-TNF agents.

• Assays for trough anti-TNF drug levels are fairly similar and interchangeable. Suggested trough levels for infliximab are ≥5 µg/mL, adalimumab ≥7.5 µg/mL, and certolizumab ≥20 µg/mL.
