About half of U.S. pregnancies are unintended, with higher rates among black women. In addition, minority women are at excess risk for HIV infection, which is primarily acquired heterosexually. In a synthesis of relevant studies (AIDS 2016; 30:2665), progestin-only injectable contraception (most commonly depot medroxyprogesterone acetate [DMPA]) compared with nonhormonal or oral contraception was associated with a 40% higher risk for HIV infection, leading the WHO to update its guidance about contraceptive eligibility: While DMPA continues to be recommended, concern about the possibility of increased risk for HIV is acknowledged.
