Researchers retrospectively evaluated the effect of transitional care nurse (TCN) assessment of a convenience sample of geriatric patients who presented to three emergency departments (EDs). The studied intervention was consultation with a TCN who evaluated patients using a variety of different functional status surveys and initiated the optimal disposition and level of needed support. Patients who were not evaluated by the TCN formed the control group. Post hoc statistical analyses were designed to adjust for possible confounding variables. The primary outcome was rate of hospital admission at the index visit. Secondary outcomes were the rate of ED revisits within 72 hours and the rate of 30-day hospital readmission.
