As we survey the literature for NEJM Journal Watch, we sometimes encounter research and guidelines that illuminate or provoke controversies in clinical medicine. In the past, we've occasionally addressed such topics in essays that are a bit longer — and provide more depth — than our typical summaries. Moving forward, we plan to publish these pieces more frequently, under the name “Clinical Spotlight.” The latest piece is “Steroids and Saline in the ICU: One Critical Care Physician's Perspective.”
