Intracerebral hemorrhage (ICH) is a serious form of stroke, with higher mortality compared with ischemic stroke. Whereas ischemic stroke can be treated with thrombolysis, there are no proven pharmacologic treatments for ICH outside of blood pressure lowering. Researchers evaluated the antifibrinolytic medication tranexamic acid for patients with ICH that could be treated within 8 hours of symptom onset. The trial was carried out over a four-year period at 124 international sites. Patients were randomized to receive tranexamic acid (1 g by intravenous bolus followed by a 1-g IV infusion over 8 hours) or placebo. The primary outcome was favorable shift on the modified Rankin scale (mRS) at 90 days between tranexamic and placebo recipients.
