The American Society for Gastrointestinal Endoscopy has proposed a 90% negative-predictive value (NPV) for optical diagnosis of diminutive adenomatous polyps in the rectosigmoid as the performance standard for allowing adoption of a “diagnose-and-leave” strategy. While several studies show that this threshold is attainable, a documented performance gap between experts and nonexperts has contributed to hampering the widespread adoption of endoscopic histologic prediction.
