Inflammatory bowel disease (IBD) is a chronic condition that is expensive to manage. The institution of a specialty medical home for these patients by at least one large academic center (University of Pittsburgh) has shown decreased costs via a reduction in ER visits and hospitalizations and improved overall patient outcomes. Now, clinicians have described an intensive medical home program established within their private community practice.
Enrolled patients are all insured by Blue Cross Blue Shield of Illinois, which has financed the project.
