Pyoderma gangrenosum affects patients with inflammatory bowel disease (IBD); few effective treatments are available. Investigators at the University of North Carolina tried oral tofacitinib in three patients with Crohn's disease (CD) and refractory PG who had failed to benefit from steroids and antitumor-necrosis-factor (anti-TNF) therapy.
Oral tofacitinib 5 mg twice daily resulted in significant improvement, if not complete healing, in all three patients by 12 weeks. In one patient, therapy was increased to 10 mg twice daily with continued improvement. Skin biopsy samples from these patients showed activated JAK1, JAK2, JAK3, and STAT1 and STAT3 on immunohistochemical staining, documenting the proof of concept for this therapy. The study wa…
