Tranexamic acid (TA) blocks fibrinolysis and has been used systemically to stop bleeding related to trauma, surgery, or other causes. Two small clinical trials of systemic use for hemoptysis yielded promising results, but inhaled administration has not been studied. Israeli investigators randomized 47 adults within 24 hours of hospital admission for nonmassive hemoptysis (i.e., ≤200 mL blood and no evidence of hemodynamic or respiratory instability) to receive either nebulized TA or nebulized normal saline (placebo) for as long as 5 inpatient days; the hospital pharmacy converted intravenous TA to the nebulized form.
