At many cancer centers, patients with pancreatic cancer are treated with external beam radiation therapy. To help precisely target pancreatic masses, gold fiducial markers can be placed directly into the target lesion via endoscopic ultrasound (EUS)–guided delivery. Historically, EUS-guided fiducial placement was performed in an ad hoc manner by backloading fiducials into EUS fine-needle aspiration needles and held in place with bone wax in an off-label manner. Recently, EUS needles preloaded with fiducials have become commercially available.
