Migraine is a common, disabling neurologic disorder. Available acute treatments, such as triptans, have significant limitations, including incomplete pain relief, high rates of pain recurrence, side effects, and cardiovascular contraindications. Calcitonin gene–related peptide (CGRP) has emerged as a key factor in migraine pathophysiology and as a therapeutic target. Six small-molecule CGRP receptor antagonists (gepants) for acute migraine treatment have been studied. Two of the gepants, including rimegepant, remain in development (and not yet FDA approved).
