South Africa has the largest HIV treatment effort of any country and is struggling with over-burdened clinics and low retention in care and adherence rates over time. A national chronic disease adherence effort is underway. For stable patients, the intervention includes use of adherence clubs (ACs; small groups that meet locally to discuss adherence and pick up prepackaged medications) and decentralized medication delivery (DMD; patients pick up medication locally, away from the clinic, at places like a school or church).
