There remains uncertainty about the value of using genotype information to guide decisions about P2Y12 inhibition in patients with ST-segment elevation myocardial infarction (STEMI) undergoing percutaneous coronary intervention (PCI) with stent implantation. In the POPular Genetics trial (NCT01761786), 2488 such patients were randomized to receive genotype-guided therapy or standard treatment with either ticagrelor or prasugrel. In the genotype-guided group, carriers of a loss-of-function CYP2C19 allele were treated with ticagrelor or prasugrel; other patients received clopidogrel according to the label instructions. Periprocedural treatment was at the clinician's discretion. All patients were treated for at least a year.
