Most cases of primary hyperparathyroidism (PHP) are diagnosed when mild hypercalcemia is noted incidentally on blood chemistry panels. An international guideline recommends that patients who are 50 or older with asymptomatic PHP undergo routine renal imaging (to detect asymptomatic nephrolithiasis or nephrocalcinosis) and bone-mineral density (BMD) testing to help clinicians decide whether to refer for parathyroidectomy (J Clin Endocrinol Metab 2014; 99:3561). In this retrospective study from an endocrine clinic in Edinburgh, Scotland, researchers assessed the yield of those tests in about 600 middle-aged and older adults with newly diagnosed primary hyperparathyroidism.
