Desmopressin for Antiplatelet-Associated Intracranial Hemorrhage?

In a retrospective study, desmopressin administration was associated with less hemorrhage expansion but no change in clinical outcomes.

Patients with intracranial hemorrhage (ICH) who are on antiplatelet agents have poor outcomes, and platelet infusions almost certainly don't help (NEJM JW Neurol Jul 2016 and Lancet 2016; 387:2605). Desmopressin may improve platelet function but is under studied.

Researchers performed a retrospective chart review of adult patients with antiplatelet-associated ICH at a single academic center. The primary outcome was ICH expansion ≥3 mL on repeat computed tomography scan within 24 hours. Exclusion criteria were no comparative scan performed within 24 hours, anticoagulation, fibrinolytic therapy, concurrent ischemic stroke, and neurosurgical intervention.
