Patients with intracranial hemorrhage (ICH) who are on antiplatelet agents have poor outcomes, and platelet infusions almost certainly don't help (NEJM JW Neurol Jul 2016 and Lancet 2016; 387:2605). Desmopressin may improve platelet function but is under studied.
Researchers performed a retrospective chart review of adult patients with antiplatelet-associated ICH at a single academic center. The primary outcome was ICH expansion ≥3 mL on repeat computed tomography scan within 24 hours. Exclusion criteria were no comparative scan performed within 24 hours, anticoagulation, fibrinolytic therapy, concurrent ischemic stroke, and neurosurgical intervention.
