The most common functional (psychogenic) neurologic disorder is tremor. Although some researchers have studied cognitive-behavioral therapy (CBT) in psychogenic nonepileptic seizures (PNES; NEJM JW Psychiatry Aug 2010 and Neurology 2010; 74:1986), recent studies on functional tremor (FT) have concentrated on motor rehabilitation. Functional neuroimaging of FT has suggested its association with differences in the anterior cingulate/paracingulate cortex. However, whether these differences correlate with treatment response is unknown. In an open-label, uncontrolled study, 15 patients with FT (mean age, 50.5; 12 women) underwent 12 weekly CBT sessions, as well as pre- and posttreatment functional MRI (fMRI) with motor and emotion-based tasks.
