A Model for Predicting Multidrug-Resistant Infections in the Emergency Department

In a large emergency department cohort, known risk factors for resistant pathogens were assigned weights and integrated into an easily derived score with distinct risk categories.

Emergency department (ED) patients often harbor multidrug-resistant organisms (MDRO). A scoring system to identify patients at high MDRO risk could be helpful. To establish such a system, researchers assessed 5460 adult patients presenting with a clinical suspicion of infection to EDs in 54 Spanish centers during 2017. Roughly 60% of the cohort served as the derivation cohort; the remaining 40%, the validation cohort.

One quarter of the patients were identified as having an MDRO infection, which was an independent risk factor for mortality (odds ratio 1.6) and for readmission (17% in non-MDRO and 23% in MDRO infections).
