Patients with biliary obstruction at the level of the ductal hilum are often difficult to treat via endoscopic retrograde cholangiopancreatography (ERCP). The decision to place a single unilateral stent (to drain the side of the biliary tree with the most severe obstruction) or two bilateral stents (to drain both the left and right biliary systems) is complex and needs to take into account tumor and ductal anatomy, relative proportions of the biliary tree to be drained by each stent, and the technical feasibility of stent placement.
