Urokinase Added to Mechanical Thrombectomy for Ischemic Stroke Treatment

Is adjunctive thrombolysis useful in acute ischemic stroke endovascular treatment?

In patients with acute ischemic stroke and large-vessel occlusion, mechanical thrombectomy (MT) improves outcomes for patients treated within 6 hours of symptom onset. Select patients can benefit from MT beyond 6 hours. In the 6-hour window, intravenous thrombolysis with alteplase is frequently administered to patients who meet the criteria. Whether intra-arterial (IA) thrombolysis is useful as an adjunct to MT is not clear.

These authors reviewed the experience of a single high-volume interventional stroke center. Over an 8-year period, 993 patients met the study criteria of treatment with MT for anterior-circulation stroke. In these patients, the median age was 75 years and 51% were women.
