Transthyretin amyloid cardiomyopathy (ATTR-CM), a condition caused by misfolded transthyretin accumulating in the heart, is gaining attention because there is now an FDA-approved drug that can improve outcomes. In 2018, a manufacturer-funded, phase 3 trial (ATTR-ACT; NCT01994889 and NEJM JW Cardiol Oct 2018 and N Engl J Med 2018; 379:1007) showed that tafamidis — which binds and stabilizes transthyretin, slowing amyloid deposition — reduces all-cause mortality and cardiovascular hospitalizations and slows decline in quality of life, compared with placebo.
