Hyperplastic polyps (HPs) <10 mm in size in the rectum or sigmoid colon are considered clinically insignificant lesions. There is, however, uncertainty regarding HPs <10 mm proximal to the sigmoid. Using data from 8560 patients in a statewide colonoscopy registry, investigators assessed the risk for metachronous large (≥1 cm) serrated polyps (SPs) and advanced adenomas (AAs; adenomas ≥1 cm or with villous elements, high-grade dysplasia, or colorectal cancer) in patients with proximal 5-to-9-mm HPs (n=376).
The adjusted odds ratios (with 95% CI in parentheses) for metachronous large SPs and AAs, respectively, excluding patients with synchronous adenomas, were (by index finding):
