Immune checkpoint inhibitors have substantial activity in patients with microsatellite instability (MSI)-high metastatic colorectal cancer (CRC). First-line use of pembrolizumab was recently shown to be superior to chemotherapy (NEJM Oncol Hematol Aug 2020). However, these agents have not shown activity in microsatellite stable (MSS) CRC.
