Endoscopic ultrasound (EUS)–guided transgastric endoscopic retrograde cholangiopancreatography (ERCP), or EDGE, is most commonly performed in patients who have undergone Roux-en-Y gastric bypass (RYGB) and require a subsequent ERCP. EDGE allows the creation of a transluminal gateway between the gastric pouch or the efferent jejunal limb and the remnant stomach using a stent, most commonly a lumen-apposing metal stent (LAMS), through which ERCP can be performed. Outcomes of EDGE were recently evaluated in a retrospective multicenter study.
Among 178 patients who underwent EDGE during a 4-year period, 13% had undergone prior failed enteroscopy-assisted ERCP. Almost all patients received a LAMS. Results were as follows:
