What Factors Influence Estradiol Levels in Women Receiving Postmenopausal Hormone Therapy?

In the ELITE trial, serum estradiol was higher among obese women and those consuming alcohol and lower among those who smoked cigarettes.

In a post hoc analysis of the ELITE trial (NEJM JW Womens Health May 2016 and N Engl J Med 2016; 374:1221), investigators explored the factors that influence circulating estradiol concentration. In the original study, 275 women who received oral micronized estradiol (1 mg daily) contributed >2000 measurements of serum estradiol over a median follow-up of 4.8 years. Mean estradiol concentrations were 11 pg/mL at baseline and 56 pg/mL while receiving treatment.

Median estradiol levels were significantly higher in women who consumed >2 alcoholic drinks daily than in those who did not drink (95 pg/mL vs. 46 pg/mL). Compared with never-smokers, current smokers had significantly lower estradiol (50 pg/mL vs. 26 pg/mL). Estradiol levels rose with i…
