Tofacitinib is the first Janus kinase (JAK) inhibitor approved for ulcerative colitis. Data from controlled trials of maintenance therapy for 52 weeks demonstrate effectiveness and a sustained response. Use is associated with an increased risk for herpes zoster, and recently a black box warning was added for an increased risk for developing venous thrombotic events with use of the higher 10-mg, twice-daily dose.
In an ad hoc analysis of an industry-sponsored, open-label extension study, investigators assessed continued efficacy and safety in 142 patients on a 5-mg, twice-daily dose for 3 years. These patients had previously achieved remission after 52 weeks of maintenance therapy.
