Healthcare managers and providers have increased interest in addressing suboptimal management of risk factors by using population-based health strategies that typically leverage remote devices and algorithm-based decision support. Now, investigators who developed a program to improve lipid and hypertension control report their experiences with the first 5000 patients enrolled in the program (mean age, 64; 55% women; 26% nonwhite).
