Based on available evidence, it is reasonably clear that patients with ischemic cardiomyopathy benefit from an implantable cardioverter-defibrillator (ICD) if their left ventricular ejection fraction (LVEF) is less than 35%. However, this is less clear for patients with nonischemic dilated cardiomyopathy. Indeed, in some studies, no benefit was observed (NEJM JW Cardiol Oct 2016 and N Engl J Med 2016; 375:1221). Scar burden, as identified by late gadolinium enhancement (LGE) on MRI, is emerging as a potent calibrator for risk stratification in many cardiomyopathies, including nonischemic dilated cardiomyopathy.
