One dilemma in treating breast cancer is how long to continue adjuvant endocrine therapy. Late recurrences can occur in patients with estrogen receptor (ER)–positive breast cancer. Research has demonstrated that 10 years of tamoxifen may be better than 5 years; so may 5 years of tamoxifen followed by 5 years of an aromatase inhibitor, with expected attrition of patients due to compliance and adverse events.
