Hospital and Postdischarge Quality Improvement Intervention in Heart Failure with Reduced Ejection Fraction

A 12-month program in hospitals did not show an effect on care quality or outcomes.

Despite an expansion in therapies for heart failure with reduced ejection fraction (HFrEF), adoption of guideline-directed medical therapy (GDMT) remains suboptimal. In this industry-sponsored, cluster-randomized trial, researchers assessed the effects of a site-level educational intervention on the use of GDMT in patients being discharged after hospitalization for HFrEF and on patient outcomes.

Investigators randomized 161 hospitals (mean bed size, 442; 95% urban), from which 5647 individuals with HFrEF who were being discharged were evaluated (mean age, 63 years; 33% women; 87% with prior heart failure).
