Multiple U.S. guidelines recommend lowering low-density lipoprotein cholesterol (LDL-C) to below 70 mg/dL in patients with high-risk atherosclerotic cardiovascular disease (ASCVD). European guidelines recommend even lower targets. In an exploratory analysis of data from the industry-funded FOURIER trial (NEJM JW Cardiol May 2017 and N Engl J Med 2017; 376:1713), investigators tested the hypothesis that lowering LDL-C to <40 mg/dL would confer additional cardiovascular benefit.
The analysis included 27,564 FOURIER participants with ASCVD; their median baseline LDL-C level was 93 mg/dL. Among participants randomized to evolocumab, 65% achieved LDL-C <40 mg/dL within a median follow-up of 2.2 years.
