Preoperative cefazolin lowers risk for prosthetic joint infection (PJI) and has become standard of care. As bacterial biofilms represent a critical component in the pathophysiology of PJI, Pant and colleagues performed a study to evaluate the anti-biofilm and anti-planktonic activities of synovial fluid against three strains of Staphylococcus aureus (one methicillin-susceptible [MSSA] and two methicillin-resistant [MRSA]). Synovial fluid was obtained from 26 patients who received cefazolin prophylaxis as part of routine total knee arthroplasty.
Cefazolin minimum inhibitory concentrations (MICs) in bacteriologic media were 1.4 mg/L (MSSA) and 23 mg/L (MRSA). Interestingly, in synovial fluid, cefazolin MICs were 0.7 mg/L for the same strains. …
