Does Timing of Dual Antiplatelet Therapy with Cilostazol After Stroke Affect Treatment Benefits?

Robust benefits of combination therapy with cilostazol were seen when treatment was started weeks or months later, but not within 8 to 14 days after the index stroke.

For secondary stroke prevention, the benefits of combination therapy with aspirin/clopidogrel or aspirin/ticagrelor over aspirin alone are concentrated in the first hours, days, and weeks after an index stroke. In contrast, the benefits of combination therapy with cilostazol (cilostazol/aspirin or cilostazol/clopidogrel; cDAPT) over aspirin or clopidogrel alone seem to develop over longer timescales that are measured in months. In the Cilostazol Stroke Prevention Study for Antiplatelet Combination (CSPS.com) randomized trial, cDAPT used for a median of 1.4 years reduced risk for recurrent stroke more than aspirin or clopidogrel alone.
