Achieving health equity and optimal outcomes for acute ischemic stroke includes an evaluation of the factors at play when a patient meets eligibility criteria for treatment with tissue plasminogen activator (tPA) but declines treatment. Using nationwide data from the Get With The Guidelines–Stroke registry, investigators identified 177,115 patients with acute ischemic stroke who presented to participating centers between January 1, 2016, and March 28, 2019, within 4.5 hours of symptom onset and were otherwise eligible for tPA. Self-reported race/ethnicity was recorded as 68.9% non-Hispanic white, 16.4% non-Hispanic Black, 7.7% Hispanic (any race), 2.9% Asian, and 4.0% other or unable to determine.
