Duration of Dual Antiplatelet Therapy Following Minor Stroke or TIA

Time-course analysis of CHANCE-2 trial data suggests that patients with impaired clopidogrel metabolism benefit most from DAPT in the first three weeks.

Recent studies that have examined dual antiplatelet therapy (DAPT) following minor stroke or transient ischemic attack (TIA) have followed patients for 90 days. However, the observation was made with aspirin and clopidogrel (A+C) that 3 weeks of DAPT treatment may be optimal to balance prevention of ischemic events and avoidance of major bleeding. The optimal duration of aspirin plus ticagrelor (A+T) is uncertain. These authors analyzed data from the CHANCE-2 trial, in which patients with minor stroke or TIA were assessed for loss-of-function alleles that could impair clopidogrel metabolism.
