Tecovirimat (TPOXX) is FDA approved in adults and children for treatment of smallpox, which is caused by an Orthopoxvirus related to Variola virus. Tecovirimat's mechanism of action is through inhibition of p37, a conserved protein in Orthopoxviruses (including Monkeypox virus) that mediates the formation of extracellular enveloped virions. This drug's efficacy against Orthopoxviruses was established based on reduced mortality in nonhuman primates and rabbits. A recent case series of U.K. patients infected with Monkeypox virus indicates that tecovirimat shortens illness duration.
