Digital health interventions may provide opportunities for more-efficient, algorithm-based risk factor management outside of the healthcare setting. In this study, a group of investigators from an academic hospital implemented a remote healthcare program aimed at improving guideline-directed blood pressure and cholesterol control.
From 2018 to 2021, they enrolled 10,803 patients (3658 with hypertension, 8103 with elevated cholesterol, and 958 with both) from within an integrated health network, identifying participants through the electronic health record or clinician referral.
