Extracranial-to-intracranial (ECIC) artery bypass surgery has been proposed as a method to improve brain perfusion and prevent stroke. However, two major clinical trials in recent decades have not shown definite benefit with ECIC surgery in patients with internal carotid artery (ICA) or middle cerebral artery (MCA) occlusion. These investigators conducted a randomized trial at 13 specialized centers in China to compare ECIC bypass surgery plus medical therapy with medical therapy alone. Patients were 18 to 65 years old and had an ICA or MCA occlusion and a history of stroke or transient ischemic attack (TIA) in the past 12 months.
