Although nonpharmacological interventions are the mainstay of managing patients with postoperative delirium, antipsychotics often are used to control agitation. Over time, use of atypical antipsychotics has increased because of the perception that they are less harmful than haloperidol. However, the comparative safety of antipsychotics in patients with delirium is unclear. Researchers used an inpatient database of more than 17,000 older patients (age, ≥65) without psychiatric histories who were prescribed oral antipsychotics (i.e., haloperidol, olanzapine, quetiapine, or risperidone) within 7 days after undergoing major surgery.
