Ketogenic diet therapies (KDTs) are high-fat, low carbohydrate diets that can improve seizure response in children and adults with drug-resistant epilepsy. Rigorous evidence supporting safety and efficacy of KDTs in children under age 2 is limited. To identify whether use of the classic ketogenic diet is superior to standard pharmacological treatment in infants with drug-resistant epilepsy, 136 infants (aged 1–24 months) were randomized to receive the classic ketogenic diet (CKD) or antiseizure medication (ASM) in a multicenter U.K. trial. Research personnel were masked to treatment allocation. Investigators evaluated seizure response at 6 to 8 weeks compared with baseline.
